The MomCenter Member Survey
Contest Mechanics:

1. MomCenter Philippine members who complete the survey will be qualified to win a grand prize of a 3-day, 2-night stay at the Splash Oasis Hotel.

2. Ten members who complete the survey AND invite 20 other people to COMPLETE the survey will win a voucher from SM. Winners will be randomly drawn every week by MomCenter.

3. Members can join the contest by simply answering the survey. No joining application forms required.

4. Contest will start on June 29, 2009 and will end on August 29, 2009. Announcement of winners will be done via a newsletter after a week.
